You need to know well your audience and create it to fit their needs. There are many bad examples of sites that are inaccessible - ones that use frames, flash or a large number of untagged graphics.

Content is the most important part of a website., this may seem a really obvious statement to make but it has been overlooked many, many times. Make sure you've got it right.
Don't jump ahead
Don't go live right away. Make sure you test enough your site. You should get the errors and fixe it not your visitors.
Make sure it's search engine friendly: You want people to find your website, so you need to build it search engine friendly. Keep in mind that the search engine rules change very often. So, you need to be well informed.
Good design and information:
Website has to be really attractive and have good information. If you don't catch a visitor's attention in the first 5 seconds, he'll leave your website and go to your competition. Remember: your competition is one click away.
Functionality:
Website has to be functional and really easy to understand. That old rule " less is more" applies here.
Navigation:
Navigation has to be very easy so visitors get familiar in the first 10 seconds they are visiting your website.
Balance:
It needs to have a balance between copy and graphics. Otherwise its too boring with too much copy or it kills visitors eyes with too many graphics and colors.
